(HYBRID) Licensed Vocational Nurse (LVN) – Care Navigator, Orthopedics ($87K–$90K) New

Location: Hybrid – Dallas, TX (downtown)

• Onsite: Monday, Tuesday, Thursday
• Remote: Wednesday, Friday
• The few couple of months will be fully onsite and then it transition to this hybrid schedule

️ Training Schedule & Location:

• First 1–2 weeks will be fully onsite for training
• Includes computer system onboarding and nurse shadowing
• Shadowing may take place onsite or remotely, depending on the trainer’s location

⏰ Schedule:

• Monday–Friday, 8:00 AM – 5:00 PM (with some flexibility)

Salary: $87,000–$90,000

Role Overview:

• Build trusting relationships with patients and caregivers by addressing communication and health literacy needs across all phases of care
• Assess and help resolve barriers to accessing treatment and services
• Educate patients and families on medical conditions, treatment goals, side effects, and available resources
• Provide timely referrals to local and national support services
• Coordinate care by assisting with appointments, transitions between facilities, and overall care navigation
• Gather and review medical records and information to support quality metrics and program improvement
• Document all patient education and interactions appropriately
• Communicate effectively with the healthcare team regarding patient and family needs or concerns
• Alert leadership to any issues related to a patient’s treatment or care process
• Deliver compassionate, well-organized, and interdisciplinary care in a timely and professional manner

Requirements:

• Active Texas LVN license that's in good standing
• Experience in orthopedic nursing
• Proficiency in Windows and Microsoft Office
